Preparing for your Stay

We want patients and visitors to feel a sense of ease and comfort from the moment you pass through our doors. Let us guide you through preparations, recovery, financial management and more. We are here for you, every step of the way.

Surgical Patients

Please follow all of your physician’s instructions. Your pre-operation information packet will contain contact information and further details to help you prepare for your procedure.

Lab and Imaging Patients

Please arrive at the facility 15 minutes before your scheduled appointment time; walk-in patients should also verify whether precertification is required for testing. Your physician will let you know if there are any special preparations or dietary instructions to consider the day before or the day of your appointment.

Registration

During the registration process, our friendly desk staff will confirm details regarding your procedure and also verify your insurance. You will be asked for the following when you arrive at the registration desk:

  • Photo ID
  • Primary Insurance Card
  • Secondary Insurance Card (if applicable)

Also, know that your insurance company may have set admission requirements for you and/or your treating physician. These requirements may include pre-authorization or physician referral and, if not met, could affect or eliminate your reimbursement.

Financial Assistance

We understand that the cost for medical care can be costly. Our patient representatives are available to help you manage the expense related to your care by researching your eligibility for various types of support, devising a payment plan, and reviewing applications for financial assistance. In addition to outside sources of support, Teton Valley Health may provide reduced fees or charitable funding through our Community Assistance Program.

Medical Records Release Form

Your medical records may be given to you for personal access and/or transferred to other healthcare facilities. To request your medical records, simply ask admissions staff or your nurse. You’ll be given a form to review and sign. For added convenience, you may request your records online through use of our patient portal or here:

Visitor Information

Visiting hours are dependent on the patient’s condition, and visits may be restricted at the discretion of the charge nurse. Make sure you’re healthy before planning a patient visit. If you’re experiencing nausea, coughing, sneezing, unexplained rash, or other symptoms of illness, please do not enter patient areas.

Meals are provided for our patients by the Dietary Department based on the physician’s orders. If you would like to have a meal while visiting hospitalized family member(s), you may make your request at the nurses’ station and they will be happy to order for you.

Going Home

Discharge Planning

Our discharge planner will provide you with information about home health care, medical equipment, skilled nursing facilities, transportation, Meals on Wheels, and other community resources. Assistance with questions regarding Medicare, Medicaid, Social Security, State Disability and Workers’ Compensation can also be provided.

At Home

A responsible adult needs to be available to provide transportation to your home. If this is not possible, Teton Valley Hospital will make every effort to help you coordinate your transportation. If you’re having trouble finding transportation, please tell your nurse.

If a visiting nurse or home health professional has been arranged to visit you in your home, you will be informed of this by the hospital discharge planner. If home care services are not provided as planned, please call 208.354.2383 and let the discharge planner know so that further arrangements can be made. While at home, if you feel concerned about any medical condition, contact your physician immediately.

Our Emergency Department is open 24 hours every day. In the event of an emergency, call 9-1-1 for help.
