Teton Valley Home Care Services provides care to patients and their families in the vast rural area from Swan Valley to Felt in southeast Idaho. Nurses work out of Driggs, and they travel from 30 to 150 miles each day to serve patients in an area that spans Teton Valley and parts of Fremont Counties. Home care services are vital in our rural area, where some patients would otherwise have to choose between either receiving no care, or driving many miles for care. Teton Valley Home Care services also enable many patients to stay at home rather than living for lengthy periods of time in a long-term care facility.

Home Health Care

Home Health Care provides medical services in your home, under the
direction of your personal physician. This compassionate care promotes
independence and enhances healing and comfort. Home Health Services
include:
- Intermittent Skilled Nursing Aids
- Medical Social Services
- Physical, Speech & Language Therapy
- Occupational Therapy
- Intermittent Certified Nursing
- Nutritional Counseling
- Bereavement Counseling

Hospice

Hospice helps terminally ill patients continue to live with independence
and as free of pain as possible. Families are provided physical,
emotional and spiritual support to better care for their loved
ones, most often at home.
Trained staff and volunteers serve as an interdisciplinary
team with your personal physician, evaluating specific needs and
establishing an individualized plan of care.
- Medical Director coordinates care as ordered
by your personal
physician.
- Registered Nurses coordinate all aspects of care,
with management of pain and other symptoms to help patients
remain alert and
comfortable.
- Certified Nursing Assistants help with personal care such as
bathing, exercise, grooming and diet.
- Medical Social Worker provides supportive counseling, bereavement
care and follow up. Identifies resources for legal and financial
aid.
- Physical and Occupational Therapists provide home safety
evaluations, education and pain management.
- Trained Hospice Volunteers offer comfort and support to patients
and family members.
- Hospice Chaplains help meet spiritual needs.
